HOME DELIVERED Meals Program

The Meals on Wheels Sacramento County Home-Delivered Meals Program provides nutritious, meals to Sacramento County seniors aged 60 and above who are homebound due to illness, recovery from hospitalization or surgery or other causes that prevent them from safely leaving their home to grocery shop for or prepare food on their own.

The program supports seniors in maintaining independence and remaining in their homes by combining meal delivery, wellness checks, and social support.

Participants receive a weekly delivery of frozen meals that have been designed by a Registered Dietitian to meet the daily nutritional needs of seniors.

In addition to food, delivery drivers provide regular safety and wellness check-ins and Meals on Wheels Case Managers provide program participants additional community resources as needed.

Eligibility Criteria for Home Delivered Meals

  • 60 years of age or older
  • Reside in Sacramento County
  • Primarily homebound or unable to leave home without assistance
  • Unable to prepare meals or grocery shop independently
  • Recent hospitalization or ongoing, chronic health problems
  • Lack support and assistance for daily activities
  • Live alone or with a caregiver

The Home Delivered Meals Program prioritizes individuals who are considered “most at risk,” meaning those whose health and safety could decline without regular access to nutritious meals and support services.

Weekly Meal Delivery Includes

  • One box of five frozen meals
  • Monthly newsletter/menu filled with nutrition and health tips
  • Meal heating instructions magnet
  • Weekly status check during delivery to ensure wellbeing
  • AniMeals pet food delivery at time of meal delivery (if needed)

Participants in the Home Delivery Meal Program receive Case Manager coordinated support services including referrals and additional assistance as needed. Ongoing eligibility reassessments are conducted to match participant’s changing needs.

THE PROCESS

1. Complete telephone screening/application
2. Application is forwarded to Case Management department
3. A Case Manager will contact applicant and schedule a home visit
4. Applicant will be placed on wait list*

*A waitlist is in effect due to high demand. Services are prioritized based on need with a focus on supporting the most vulnerable individuals first. We appreciate your patience.
