Meals on Wheels Case Management
Meals on Wheels Case Management is a personalized, holistic approach to assess, coordinate, and monitor the nutritional and social support needs of the participants of the Home-Delivered Meals program.
Case Managers are trained human service professionals who work directly with participants through in-home visits and regular check-ins to assess their needs. These assessments look beyond nutrition to identify challenges related to health, housing, safety, and daily living.
Case Managers identify and address unmet needs and connect participant to appropriate resources to help prevent unnecessary institutional care.
The goal is to ensure that participants receive adequate nutrition and have access to resources that promote their overall well-being, maintain their independence, health, and safety to enable them to age in place, in the comfort of their own home.
Case Management services typically include:
- Comprehensive Needs Assessment. Screenings to determine eligibility for meals and other resources and support, including wellness and safety evaluations including fall prevention, emergency response support, and more.
- Referrals to Community Resources. Based on individual need, participants are provided information and referrals to various community programs such as utility assistance, medical services, benefits programs, and more.
- Ongoing Monitoring/Follow-Up. To ensure participants needs are being met, Case Managers conduct periodic follow-up appointments, and adjust services as needed.
